If any of your employees are making any changes to their healthcare, one of these forms is required to begin the process. Please call the Bishop's Office with any questions you may have. 800-442-6918. ALL hourly employee working 20+ hours per week and any salary employee is entitled to medical, dental, and life insurance. Lay employees are also entitled to short-term and long-term disability insurance (clerical employees have this insurance through their pensions). New hire:
All new employees working 20 hours a week or more and all salaried employees must be given the enrollment packet. The New Hire Form above must be filled out for all new employees.
Benefit termination:
In order to terminate benefits the above form must be filled out. Until this form is received by Thea Mangels no change will be made, and your church will be liable for any cost that may incur.
